Backflow incident
An event in which contaminated or non-potable water actually enters a potable water system through a cross-connection.
A backflow incident is an actual contamination event: a moment when reversal conditions and an unprotected or failed cross-connection line up, and non-potable water, chemicals, or other substances enter potable piping. Incidents range from localized events affecting a single building's fixtures to more serious ones where contaminants reach the service line and the public distribution system. Discovery often begins with a customer noticing unusual color, odor, taste, or visible material in the water.
Incidents matter because they are the failure mode the entire discipline exists to prevent, and because their consequences extend beyond water quality. A suspected incident can trigger use advisories, interrupt business operations, require extensive flushing and testing before the water is cleared, and expose the responsible property to liability. Even a small incident tends to prompt a hard look at the property's protection, and often at the purveyor's wider program as well.
Testers and property owners should know the response pattern. A suspected incident should be reported to the water purveyor promptly so it can isolate the affected area, investigate the source, and direct flushing and sampling until quality is confirmed. Afterward, the contributing cross-connection is corrected and protection upgraded as needed. Testers may be called in to inspect and test assemblies as part of the investigation, and their records of prior tests frequently become important evidence.
